Our Congregate Living Facilities are looking for compassionate nurses ( LVN s or RN s ) that preferably have SNF or Sub-Acute Experience willing to do 12-hour day or night shifts. (7-7)
Our facilities are licensed by the Department of Health for 6 beds per facility and offer a more intense level of care to our patients. Our staff to patient ratio is a minimum of 1:3 Nurse to Patient Ratio and initiate as 1 on 1 level of care. Our boutique facility allows us spend quality time with our patients which allows us to be more in tune with our their physical and mental needs. We prefer to hire nurses with post acute experience staff of minimum one (1) year, but if you have the passion and heart for nursing continue reading.
Experience with Sub-acute level of patients and Vent/trach certification is preferred.
LVN ( Licensed Vocational Nurse):
Provides direct patient care as defined in California Nurse Practice Act.
Performs direct nursing care to include AM/ PM care; skin, bowel, bladder programs, nutrition and hydration, medications and ordered treatments, transfers, NG tube and gastrostomy tube feedings; positioning and wheelchair safety.
Implements plan of care initiated by the registered nurse.
Provides accurate and timely documentation consistent with the plan of care.
Participates in patient and family education, appropriately reporting the identified needs for other disciplines (OT, PT, MSW, ST, Dietitian) to the registered nurse and/or Clinical Supervisor.
Utilizes all available resources in planning care and consults as needed with RNâ€™s, medical staff, outside specialists or consultants, and other members of the interdisciplinary team.
Continuously reviews, evaluates, and documents the effectiveness of nursing care, and accomplishment of client goals for discharge.
Uses equipment and supplies effectively and efficiently.
Participates in personal and professional growth and development.
Performs other duties as assigned by the registered nurse and/or Clinical Supervisor.
Strives for excellence in client care and works to coordinate with other disciplines to ensure client safety and well being.
Current California LVN license and CPR certification (American Heart Association only), with the minimum of two years nursing care experience in sub-acute or CLHF settings. Provides care in accordance with treatment plan and professional nursing practices. Knowledge of assessment and documentation, spinal cord injury, wound care and staging, g-tube placement and care, tracheostomy care and ventilator experience. Vent/Trach & IV Certifications are preferred.
ABOUT CONGREGATE LIVING HEALTH FACILITIES:
The best way to describe congregate living facilities would be a hybrid between Sub-Acute & a Board and Care.
The setup/arrangements of our operation looks very similar to a Board and Care while the internal workings are of a sub-acute level facility. Dealing with Vent/Trach patients, Rehab patients and much higher levels of care needs with a maximum of 6 patients in a cozy homelike setting. Congregate Homes / Facilities are funded by private insurances, hospitals, Medi-Cal waivers & Privately. We are one of the newest facility types to appear in the spectrum of healthcare. Our facilities have large contract with some of the well known insurance providers and Hospitals within southern California. We follow Title-22 just like SNF's and other healthcare providers.
Our Congregate facilities take a holistic approach with each and every patient and we make sure to meet the patient where they need to be met. We like to customize our care-plans individually to each patient's needs. Due to the size of the operation we are able to be flexible and creative with the care needs and wants of our patients and there families.
