Guardian Angel Home Health Agency, Inc. is seeking an RN's & LVN's to provide a full range of in-home visits to our patients. These individuals must have bedside manners as they will be a crucial aspect of our company with face to face visits. We are looking for compassionate nurses who are passionate about their work and are proud to be a part of our Home Health team!

Our Nurses will plan, organize, and direct home care services in coordination with the Plan of Care and as specified per Conditions of Participation.


• Completes an initial assessment of patient and family to determine home care needs. Provides a complete
physical assessment and history of current and previous illness(es)
• Regularly re-evaluates patient nursing needs
• Initiates the plan of care and makes necessary revision as patient status and needs change
• Uses health assessment data to determine nursing diagnosis
• Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic,
preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and
treatments as prescribed by the physician
• Counsels the patient and family in meeting nursing and related needs
• Provides health care instructions to the patient as appropriate per assessment and plan of care
• Identifies discharge planning needs as a part of the care plan development and implements prior to discharge
of the patient


The Licensed Vocational Nurse (LVN) is responsible for providing skilled nursing care to patients in their homes under the supervision of a Registered Nurse. This LVN will follow the plan of care; provide treatments, including wound care, working collaboratively with members of the team to help exceed patient outcomes. Will provide direct patient care and family education and report identified needs to the interdisciplinary group. Must be able to demonstrate competency in nursing and assessment skills and be able to accurately identify changes of condition.

Qualifications and Skills
• Must have a current license to practice professional nursing in the State of California.
• Maintain a current CPR certification
• Must be a licensed driver with an automobile that is insured in accordance with California State law.
• Minimum of 2 years’ experience, at least 1 year of which is in the area of public health or home care nursing
is preferred!
• Self-directed and able to work in the field with minimal supervision.
• Demonstrates EXCELLENT observation, problem solving, verbal and written communication skills; nursing
skills per competency checklist.
• Shows ability to organize and prioritize workload independently.
• Prolonged or considerable walking or standing, able to lift position and or transfer patients.
• Ability to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, or crouching.
Visual acuity and hearing to perform required nursing skills.
• Bilingual (Spanish and or Russian speaking) preferred but NOT required.
