The Service Coordinator position provides coordination of services to individuals enrolled in an Adult Community Services (ACS) Program. The Service Coordinator supports the mission of The Coordinating Center by providing person-centered coordination services and adhering to the parameters of the Community Options Program and Center. Service Coordinators are responsible for complying with specific contract requirements, integrating technology to maintain database integrity, report writing and maintenance of documentation. The Service Coordinators work interdependently with the Adult Community Service Team to maximize quality outcomes for individuals. This position will be primarily focused in Montgomery County, with the flexibility to drive to surrounding counties as needed.
-Serves as the primary point of contact with clients. Performs on-site visits, provides support planning, collects information to assist in the development and monitoring of plans of service, and completes telephonic care coordination.
-Secures information regarding medical, psychological, and social factors contributing to the client's situation, and evaluates the client's abilities to assist in the development of plans of service and coordination services.
-Provides consistent telephone contact with client, family members, and others to assist with the coordination of services per The Center's protocol and contract requirements.
-Works in conjunction with ACS Consultants to meet the needs of complex clients.
-Promotes positive health behaviors to support the client's optimal health and well-being.
-Accesses and records client and community resource information and coordination activity in multiple computerized systems.
-Meets The Center's and contractual billing guidelines when documenting activities.
-Manages multiple priorities and tasks with flexibility to enhance service coordination services.
-Assists with mentoring and training newly hired Service Coordinators.
-Attends regular staff meetings and in-services.
-Identifies appropriate internal and external resources for assigned clients and works collaboratively with patients and community agencies to achieve the desired client outcomes.
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required:
-Bachelor's degree (B. A.) from four-year college or university in health/human services or related field.
-One to two years related experience and/or training and working knowledge of housing, social service and individual support services; Knowledge of Medicaid services or Home and Community Based Waivers is desirable.
-Ability to communicate effectively with individuals with varied cognitive abilities to establish professional relationships. Ability to read and interpret documents such as employment paperwork, assessment reports or medical records, and procedure manuals. -Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or co-workers of the organization.
-Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
-Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
-Ability to travel throughout the state of Maryland estimated to average about 50% of work schedule
-Ability to organize information quickly and effectively; prioritize and complete multiple tasks effectively.
-Ability to represent the organization in a positive and professional manner at all times.
